Please wear surgical or cloth face covering without exhalation valves. You must also stay 6 feet apart while on testing grounds. This is strictly enforced and you will be turned away if you do not adhere by our guidelines. 

You must bring a United States government issued ID card and medical insurance card (if you have insurance).

Who qualifies?

Every Bay Area resident over the age of 18 or minors accompanied by a parent or guardian.

How it works

  • 1.
    Select a test site and available time. Fill out the registration form.
  • 2.
    Insurance card and proof of identity are required, as well as test confirmation email or text message. Be sure to have your mobile phone in case we need to reach you.
  • 3.
    When you arrive you will see signage for the COVID-19 testing site.
  • 4.
    Your sample is sent to one of our lab partners. We'll email you a link to activate your OnPatient account, which will be used to send your results.
  • 5.
    You will receive a link to view your results within 2-3 days. Results are viewed through OnPatient

What type of test is performed?

PCR testing which has a projected turn around time for results of 3-5 days

Do you offer the rapid antigen test?

We are offering rapid antigen testing for all patients who are experiencing symptoms of COVID-19 or essential workers. This test has a projected turn around time for results of 15-20 minutes.

How is my sample for testing collected?

Our staff is highly trained to make the process as comfortable as possible. The test is done by inserting a swab into your nostril approximately 1 inch into the nasal passage (midway) with the most visible drainage or the nostril that is most congested if drainage is not present. The swab is rotated several times, removed gently and this process is repeated on the other side.

How and when will I get results?

You will be tested in one of two ways. Rapid-result testing is generally returned in person within 15-30 minutes of specimen collection. COVID-19 lab tests are sent to one of our lab partners, which may return results within 2-3 days in times of peak demand. When your result is ready, you will receive an email with a link to the OnPatient website.

Is there an out-of-pocket cost?

Patients should not have any out-of-pocket costs, but you should check with your health plan to confirm before scheduling a test. If you have health insurance, you must bring your card with you to the test site. If you do not have insurance, we will ask you for your Social Security number (card not needed), driver’s license or state ID to submit the cost of your test to the federal program for the uninsured.

What happens my results are positive?

You will receive a phone call from CityHealth and your information will be shared with the county Department of Public Health, who will assign a contact tracer to interview you. What happens if my result is negative?

You MUST continue to shelter-in-place and take every precaution not to become infected as well as to not potentially infect others. Your result will be sent to your through our HIPAA compliant health portal at Please know that a negative result only provides a snapshot in time to determine that at the time of the test, you did not have detectable levels of the virus. This does not mean that within hours of getting the test or by the time that you receive results that you won’t be infected with COVID-19.

Who does the DPH recommend for testing?

  • Anyone experiencing COVID-19 symptoms (see below)
  • Anyone, regardless of symptoms, who has been exposed to someone confirmed to have COVID-19 within 7-10 days ago
  • Anyone who has participated in protests.
  • All essential and frontline workers (grocery store, restaurant, delivery drivers, security personnel, construction, etc.)
  • All first responders (Firefighters, EMTs, Police, etc.)
  • All healthcare workers Anyone with chronic medical problems that impact the immune system

What are the symptoms of COVID-19?

There is a wide range of symptoms reported – ranging from mild symptoms to severe illness. Symptoms may appear 2-14 days after exposure to the virus. Some of the common COVID-19 symptoms include:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

What do I do if i have a COVID-19 emergency?

If you or anyone else is exhibiting any of the emergency warning signs listed below, seek emergency medical care immediately by calling 911:

  • Trouble breathing
  • Bluish lips or face
  • Persistent pain or pressure in the chest
  • New confusion
  • Inability to wake or stay awake
